The definition of "Net worth" is given under Section 2(57) of the Companies Act, 2013.
Here is a template of net worth certificate from a Chartered Accountant (CA).
The certificate can be given by an individual CA in practice or by a CA who is a partner in a CA firm.
The certificate has to be printed on the letterhead of the Chartered Accountant and duly signed and sealed by him.
