While applying for the passport of a minor child, there are certain documents to be submitted with the regional passport office.
The format of such a declaration is prescribed in Annexure-D and is available in the official passport website of the Ministry of External Affairs, Govt. of India.
The declaration is to be printed on a plain paper and duly signed by the parents or legal guardian of the minor child applicant.
